FOR LIFE CODE OF CONDUCT

For Life Pty Ltd (ACN 149 645 885) producer of the For Life Online Platform product (“For Life”, “we”, “our” or “us”) is committed to providing a safe and respectful Online Community for all involved.

It is of the utmost importance that all those that access the For Life Online Community respect and uphold the rights of other Users, including persons ageing or with disabilities to have independence, safety and quality of life. Also, that all within the Online Community uphold the rights of every care provider to a safe and professional environment. This means promoting dignity, choice, control, respect, professionalism, health and safety, open communication, ethical behaviour and freedom for all.

  1. Definitions
    1. Account means a profile registered with For Life.
    2. Account Delegate means Users who are authorised to manage the Account of Customers  on their behalf.
    3. Admin Personnel means Users who are employed by For Life to undertake Account monitoring and verification tasks on our Site.
    4. Adverse Event is a negative consequence of care that results in unintended injury or illness that may or may not have been preventable.
    5. Customers or For Life Customer means Users who register an Account to obtain care and support services  available on our Site.
    6. Disclaimer means the disclaimer published on the Site, as amended from time to time and can be accessed here.
    7. For Life means both For Life Pty Ltd and the For Life online platform.
    8. For Life Online Community or Online Community means all Users of our Site; being, Admin Personnel, Account Delegates, Customers, Providers and Visitors.
    9. Payment Terms mean the terms published on the Site relating to payment for the use of the Services, as amended from time to time and can be accessed here.
    10. Policies mean the User Agreement, Privacy Policy, Payment Terms, Disclaimer and any other terms or conditions published on the Site relating to access to the Site and use of the care and support services offered on the Site, as amended from time to time.
  2. Privacy Policy means our privacy policy published on the Site, as amended from time to time and can be accessed here.
  3. Providers or For Life Providers means Users who register an Account on our Site to deliver care and support services to Customers.
  4. Site means the For Life website located at www.forlife.com.au.
  5. Users means any person who accesses, registers and or logs onto our Site including Account Delegates, Customers, Providers and Visitors.
  6. User Agreement means the agreement between For Life and each User of our Site which sets out the terms and conditions of use of our Site, as amended from time to time and is located at www.forlife.com.au/useragreement.
  7. Visitors means Users who access our Site but do not create an Account as a Customer or Provider.

As a member of our For Life Online Community you have registered to connect with care and support services offered by For Life Providers. Whilst we appreciate you are choosing a service that promotes control, choice and self-direction or self-management, this also means you will be expected to be responsible for your choices. As such, this means you agree to the following:

  1. Communication
    1. Ensuring all written and/or verbal communication, whether via our Site or in the physical presence of others, is respectful, open and honest (whilst remaining in line with confidentiality and privacy). We expect you will communicate in a way that is professional and courteous and uses appropriate language.
  2. Compliance
    1. Everyone in the Online Community agrees to, and is expected to, read and act in accordance with our Policies, including this Code of Conduct Policy and with State and National employment and health and safety laws.
  3. Health & Safety
    1. You must:
      1. Report unsafe environments, hazards and, should they occur, Adverse Events;
      2. Take reasonable care in all activities you conduct and all professional interactions;
      3. Promote and maintain a safe work environment and culture;
      4. Uphold all obligations and responsibilities under the applicable health and safety laws, including precautionary measures to minimise injury, hazards and risks; and
      5. Ensure you always act with care to maintain your wellbeing and the wellbeing of others.
  4. Media & Social Media
    1. We believe in the power of social media in promoting self-expression and advocacy but there are some things we will expect of you. You agree that:
      1. You will not make any public comment or statement either on social media or to the wider media, where it could be construed that you are speaking on behalf of For Life or our Site.
      2. You will not share any information via social media which will be a breach of confidentiality or privacy.
  5. Privacy, Confidentiality & Information
    1. To allow For Life and For Life Providers to deliver the highest quality of support you agree to:
      1. Provide For Life and the For Life Provider with all required information relating to care and support services required, medication requirements, diagnosis, and more as needed.
      2. Ensure all information provided is current and accurate, including, but not limited to:
        1. Assessments;
        2. Care Plans;
        3. Medical or other relevant referrals; and or
        4. Medical history.
      3. Uphold the right to privacy of your For Life Provider by not taking advantage of your access to that For Life Provider’s information or disclosing their personal information to other parties.
  6. Professional Conduct
    1. At all times, you will:
      1. Be professional and ethical in your conduct with the For Life Online Community.
      2. Act to protect the professional relationship between yourself, your family, your carers, friends and/or delegates (including your Account Delegate, if nominated) and your For Life Provider.
      3. Not engage in any inappropriate behaviour, such as enter into any physical relationship, nor exploit the For Life Provider.
      4. Not act in any way which may be perceived as inappropriate or expose your For Life Provider to any material which may be vulgar, graphic, sexually explicit, pornographic, culturally insensitive or otherwise offensive or inappropriate.
      5. Report any incident or event. 
  7. Respect
    1. At all times, you are expected and agree to:
      1. Respect your For Life Provider’s background and their personal preferences (such as their cultural or religious preferences).
      2. Respect your For Life Provider’s right to privacy and freedom from inappropriate conduct.
      3. Always act with integrity and treat all members of the For Life Online Community with respect and dignity.

ARE YOU A FOR LIFE ACCOUNT DELEGATE?

As a member of our For Life Online Community you have been nominated as an Account Delegate by a For Life Customer. The For Life Customer has nominated you to assist in the registration and management of their Account to connect with care and support offered by For Life Providers. As such, this means you agree to the following:

 

  1. Accountability
    1. Uphold the best interests of the For Life Customer that has appointed you as their Account Delegate and only honestly represent and provide for their needs with their consent.
    2. You will not act in a manner that is contrary to the interests and values of For Life and the For Life Online Community.
  2. Communication
    1. Ensuring all written and/or verbal communication, whether via our Site or in the physical presence of others, is respectful, open and honest (whilst remaining in line with confidentiality and privacy). We expect you will communicate in a way that is professional and courteous and uses appropriate language.
  3. Compliance
    1. Everyone in the Online Community agrees to, and is expected to, read and act in accordance with our Policies, including this Code of Conduct Policy and with State and National employment and health and safety laws.
  4. Health & Safety
    1. You must:

    1. You will not enter into transactions that may result in a conflict of interest between yourself and the For Life Customer for whom you’ve been appointed.

    1. We trust all members of our For Life Online Community to act truthfully and honestly whilst upholding the values of empathy, choice, control, independence and quality of life.
  7. Media & Social Media
    1. We believe in the power of social media in promoting self-expression and advocacy but there are some things we will expect of you. You agree that:
      1. You will not make any public comment and statement either on social media or to the wider media, where it could be construed that you are speaking on behalf of For Life or our Site.
      2. You will not share any information via social media which will be a breach of confidentiality or privacy.

      1. Respect your For Life Customer’s & For Life Provider’s backgrounds and their personal preferences (such as their cultural or religious preferences).
      2. Respect your For Life Customer’s & For Life Provider’s right to privacy and freedom from inappropriate conduct.
      3. Always act with integrity and treat all members of the For Life Online Community with respect and dignity.

ARE YOU A FOR LIFE PROVIDER?

As a member of our For Life Online Community you have registered to offer your care and support services to For Life Customers using our Site. As such, this means you agree to the following:

  1. Accountability
    1. Only perform the tasks permitted by and appropriate to your qualifications and experience and do not attempt work that exceeds your skills or knowledge.
    2. You will not act in a manner that is contrary to the interests and values of For Life and the For Life Online Community.
  2. Communication
    1. Ensuring all written and or verbal communication, whether via our Site or in the physical presence of others, is respectful, open and honest (whilst remaining in line with confidentiality and privacy). We expect you will communicate in a way that is professional and courteous and uses appropriate language.
  3. Compliance
    1. Everyone in the Online Community agrees to, and is expected to, read and act in accordance with our Policies, including this Code of Conduct Policy and with State, Territory and national employment and health and safety laws.
  4. Conflicts
    1. You agree that gifts between yourself and your For Life Customer could be reasonably interpreted as inappropriate given the professional relationship and you undertake to not accept gifts and to ensure you notify your For Life Customer’s representative, carer, family, Account Delegate, or other appropriate person(s).
    2. You agree to avoid incidents that may result in a monetary issue or conflict of interest. Particularly, you will not undertake responsibilities relating to the finances of your For Life Customer and will record information relating to any such potential or actual incidents. Particularly, should such an incident arise, you will notify your For Life Customer’s representative, carer, family, Account Delegate, or other appropriate person(s).
  5. Ethics
    1. We trust all members of our For Life Online Community to act truthfully and honestly whilst upholding the values of empathy, choice, control, independence and quality of life.
    2. You will honestly record the hours you have worked in providing care and support services to your For Life Customers and understand your hours will be submitted to the For Life Customer for confirmation.
    3. You agree to ensure the information in your Account is accurate and provides current information about your skills, qualifications and experience.
  6. Health & Safety
    1. You must:
      1. Report unsafe environments, hazards and, should they occur, Adverse Events;
      2. Take reasonable care in all activities you conduct and all professional interactions;
      3. Promote and maintain a safe work environment and culture;
      4. Uphold all obligations and responsibilities under the applicable health and safety laws, including precautionary measures to minimise injury, hazards and risks; and
      5. Ensure you always act with care to maintain your wellbeing and the wellbeing of others.
  7. Media & Social Media

      2. Uphold the right to privacy of your For Life Customer by not taking advantage of your access to their information or disclosing that personal information to other parties.
  2. Professional Conduct
    1. At all times, you will:
      1. Be professional and ethical in your conduct with the For Life Online Community.
      2. Act to protect the professional relationship between yourself and your For Life Customer and your For Life Customer’s family,  carers, friends and or delegates (Account Delegate if nominated).
      3. Not engage in any inappropriate behaviour, such as enter into any physical relationship, nor exploit the For Life Customer.
      4. Not act in any way which may be perceived as inappropriate or expose your For Life Customer to any material which may be vulgar, graphic, sexually explicit, pornographic, culturally insensitive or otherwise offensive or inappropriate.
